New clients - Please have this form filled out prior to your appointment:

*Returning clients - If it has been 3+ months since your last visit, you must fill out the form again.

• I am located inside Salon Republic near Floor & Decor in Murrieta Town Center.
• Address: 39825 Alta Murrieta Dr B-25, Murrieta, CA 92563 (SUITE 707)
• When you arrive, please take a seat closest to my suite and text me (951) 973-1089 when you've arrived.
• No extra guests allowed (Minors MUST have parent/legal guardian present)
• Masks are optional during service. 
• If you are running late, I may not be able to take you for your appointment and will need to reschedule. (*$25 fee will still apply) I am not squeezing anyone in due to needing proper time to clean/sanitize my room & equipment.

Cancellation Policy

 We ask that you please reschedule or cancel at least 1 day before the beginning of your appointment or you may be charged a cancellation fee of $25.00.

No Call/No Show Policy

If you no call/no show to your appointment, you will be charged 100% of all services booked and will be required to prepay for any future services.
